Selection of thoracentesis points for thoracentesis
Jan 21, 2022
(1) Thoracic puncture and fluid extraction: first perform chest percussion and select the part with obvious solid sound for puncture, which can be located in combination with X-ray and B-ultrasound. The puncture point can be marked with nail violet on the skin. It is often selected: 7 ~ 9 intercostals of the inferior scapular line; Posterior axillary line 7 ~ 8 intercostals; Axillary midline 6 ~ 7 intercostals; Anterior axillary line 5 ~ 6 intercostals.
(2) Encapsulated pleural effusion: puncture can be performed in combination with X-ray and ultrasonic positioning.
(3) Pneumothorax aspiration and decompression: generally select the second intercostal space in the middle clavicular line or 4 ~ 5 intercostal space in the middle axillary line on the affected side. Because intercostal nerves, arteries and veins run along the lower edge of ribs, they should be punctured through the upper edge of ribs to avoid damaging nerves and blood vessels.
